Registration for EVMS

Welcome to East Valley Middle School!!!  We want to make sure that the enrollment process is as quick and easy as possible for you. If you just click on this link Boundary Map, you can type in your address and find out the school your child will be attending. STEP 1: Complete the pre-enrollment online. (Adobe Flash is required to use the site, you can download it free here.)

STEP 2: When you get ready to enroll your student, the following items are required at all Nampa Schools:

BIRTH CERTIFICATE

  1. A certified copy of the student’s birth certificate or other reliable proof of a student’s identity and birth date must be provided to the school.  We cannot accept hospital birth certificates. If other reliable proof is provided, rather than a birth certificate, that proof shall be accompanied by an affidavit explaining the inability to produce a copy of the birth certificate.  Other reliable proof of a student’s identity and birth date may include a passport, visa or other governmental documentation setting forth a child’s identity.
  2. You may contact the Vital Statistics Department of the State where your child was born if you do not have a certified birth certificate.  The following web site will give information and applications for all states.  http://www.cdc.gov/nchs/howto/w2w/w2welcom.htm
  3. If your child was born in Idaho, you may contact Health and Welfare at http://www.healthandwelfare.idaho.gov

IMMUNIZATIONS

The Idaho Immunization Law states that children of grades pre-school through 12 shall provide proof of immunization at the time of first enrollment in an Idaho school.  Your child’s immunizations must be on record for your child to be present at school.  It is the responsibility of the parent or guardian to provide proof of immunizations to the school.

ADDRESS VERIFICATION

The legal residence of the student’s parent/guardian determines the student’s designated attendance area.  Examples of acceptable address verification include telephone bill (not cell phone), electric bill, water/sewer/trash bill, hook-up receipt to any of the aforementioned, rental/lease/purchase agreement. Not acceptable: car registration, drivers license or car insurance.

The bill must show a recent date (within 30 days) and parent or legal guardian’s name and address.

NATURAL PARENT OR LEGAL GUARDIAN

Natural parent must be present to enroll student or a guardian with legal guardianship papers.

DIVORCED PARENTS

Please bring a copy of any divorce decree or custody agreement pertaining to the child.

SPECIAL SERVICES

If your child was serviced in Special Education at the prior school, please bring a copy of the current IEP and eligibility report.

ENROLLING FROM OUT OF OUR DISTRICT

Please bring the child’s recent transcript and/or report card.
